Paul- Assessment (SLP)
Paul is a 63-year-old male who incurred an ischemic stroke 2 years ago. It is now 2 years after his stroke and he has returned to the outpatient clinic for an updated evaluation. He is interested in pursuing additional therapy for working on writing skills for emailing friends and family as well as continuing to improve his verbal expression and speech to be able to talk better on the phone. You are the speech-language pathologist assigned to complete his evaluation.
Recommended time for completion is 120 minutes.

Anna- Intervention (SLP)
You are a speech-language pathology assistant and assigned to work with Anna. Anna is a 4-year-old girl with single sound errors. Please complete your intervention and then report your findings to your supervising SLP.
Recommended time for completion is 75 minutes.

Latreece- Assessment (SLP)
Latreece is in my second-grade class. She was previously retained in first grade so she is older than the other children, yet she still seems way behind them in achievement. She exhibits severe language difficulties in the classroom. She is in the Title I Reading Program however she continues to struggle. This is her first formal evaluation.
Recommended time for completion is 120 minutes.

Doug- Assessment (SLP)
Doug will be a freshman in our school this year. This summer Doug suffered a severe traumatic brain injury when he was struck in the head by a baseball. As a result of his brain injury, Doug demonstrates significant cognitive, physical, and communication deficits. Prior to his injury, Doug was reported to excel in all academic areas. He is being referred for evaluation and your involvement and recommendations are greatly appreciated.
Recommended time for completion is 120 minutes.

Molly- Assessment (SLP)
Molly is a student in my sixth-grade language arts class. She stutters when speaking and talks very fast.
Recommended time for completion is 75 minutes.

Carley- Assessment (SLP)
My daughter, Carley, is 3 years old. I requested a speech and language evaluation because I am concerned that Carley's speech and language skills may be delayed for her age.
Recommended time for completion is 75 minutes.

Colt- Assessment (SLP)
Colt was recently diagnosed with vocal nodules and his ENT recommended speech therapy.
Recommended time for completion is 75 minutes.

Oliver- Assessment (SLP)
Oliver is a 3-year-old boy with developmental delays. He received early intervention services since birth and is now transitioning to public school preschool.
Recommended time for completion is 120 minutes.

Deon- Assessment (SLP)
Deon is a 10-year-old boy who is due for his three-year re-evaluation. He is currently in a resource classroom for children with special needs. He receives speech-language services as well.
Recommended time for completion is 120 minutes.

Dora- Assessment (SLP)
Dora is a 3-year-old girl from a primarily Spanish-speaking home who was referred by her parents for an evaluation of her language skills due to very limited expressive language.
Recommended time for completion is 75 minutes.
